REPEAT MEDICATION REQUEST FORM
If your pet is on long-term medication for a chronic health condition, you can speed up a resupply of medication by sending this request form through. Please note that to satisfy the legal requirements of medication dispensation, it is possible our vets will need to contact you or schedule a visit with your pet, regardless of how long it has been since the last time your pet was checked.
We will review this form and be in contact either via phone or email, to advise you that your medication has been ordered, or to let you know it’s ready for collection, or to advise you that the vets have requested a scheduled visit or phone contact prior to dispensation.
